What you should have in your request:
-> Clan name
-> Clan tag
-> Clan games
-> Clan membership(active within past month & total)
-> Competitive or Noncompetitive
-> Officer names/leadership
-> Contact info(website, Teamspeak, Steam, etc)
-> Type of request(NAP, Alliance, other[please explain])
